Sage case management brings decades of combined experience working with Kentucky’s Michelle P and Supports for Community Living waivers. We share the mission of the waivers to support people with intellectual and developmental disabilities towards their best health, welfare, and longevity. We strive to ensure each individual has access to their community, to the relationships they want, and to pursue the same endeavors that add quality to their life experiences.
