How long has Sage been open?
Sage has been an approved waiver provider since 2013.
Does Sage specialize in any particular diagnosis?
Sage offers a broad and deep range of experience including but not limited to individuals with autism, Down syndrome, cerebral palsy, mild to profound intellectual disabilities, spina bifida, and mental health diagnoses.
How long do I have to wait before services begin? Does Sage have a waiting list?
Sage works with the individual’s team to initiate services as quickly as possible, which includes the prior authorization process. While this can vary, Sage understands the importance of providing quick attention not only to processes but in addressing the individual’s needs in real time. Delays should be minimal. We may periodically have a waiting list.
How do I select a case manager or clinician?
Sage supports freedom of choice. Whether the individual or family chooses Sage for clinical or case management support, the option of keeping or changing the provider belongs to the person/guardian. Sage is committed to meeting the individual and family’s expectations but understands that the freedom to choose belongs to them.
How can behavior supports help me/my loved one?
While each individual’s behavioral goals vary, Sage’s clinicians will provide clinical insight, caregiver training, interventions, and ongoing monitoring to assist the individual in meeting their behavioral goals. Sage clinicians will work with the family, team, and/or individual towards improved skills acquisition, independence, and a broad range of functional living skills that reinforce the same mission of assisting the individual towards their best life experiences, relationships, employment, and longevity.
Using one of Kentucky’s waivers often brings many questions, concerns, and expectations. Sage can help by offering experience that supports the individual and their family through the process together!
