Sage Support Services is a unique community-based waiver provider offering clinical and case management services to individuals with intellectual and developmental disabilities using the Michelle P (MPW) and Supports for Community Living (SCL) waivers, as well as clinical services for individuals using the Acquired Brain Injury waiver (ABI).
Sage is unique in that we are a hybrid provider. While case management is required to be a separate service from all other waiver services, the insight and partnership of housing both services maximize service delivery, whether your loved one is using case management or clinical services, such as behavior supports. Kentucky’s waivers are heavily regulated, often lending to confusion, difficulty understanding requirements, and making waiver navigation more challenging. Sage’s depth of experience through both case management and clinical services assists with this common barrier by reassuring the individual, their family, and team that they will have the support necessary to see them through those challenges smoothly.
Sage practices and promotes the power of positive partnerships. Individuals using waiver services will be supported through a team of service providers. Those partnerships will dramatically impact the progress and satisfaction of service delivery for, of, and by the individual. Sage’s clinical team and case managers bring decades of combined experience working with children, adults, and their families with developmental and intellectual disabilities.
